Job Description

The Social Worker is responsible to provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being.

Responsibilities:

  • Conduct diagnostic evaluations and assessments to determine the needs of patients
  • Develop and implement treatment plans based on patient assessments and goals
  • Provide individual and group therapy sessions using evidence-based practices such as cognitive behavioral therapy and motivational interviewing
  • Collaborate with interdisciplinary teams to coordinate care and ensure comprehensive treatment for patients
  • Maintain accurate and up-to-date medical documentation in accordance with regulatory standards
  • Participate in clinical research projects to contribute to the advancement of social work practices

Skills:

  • Knowledge of medical terminology to effectively communicate with healthcare professionals
  • Strong assessment skills to evaluate patient needs and develop appropriate treatment plans
  • Experience in conducting diagnostic evaluations to identify mental health conditions
  • Familiarity with cognitive behavioral therapy techniques for behavior modification and emotional regulation
  • Ability to engage patients using motivational interviewing techniques to promote positive change
  • Understanding of social work principles and ethics to guide professional practice
  • Excellent written and verbal communication skills for effective collaboration with interdisciplinary teams
  • Attention to detail in maintaining accurate and thorough medical documentation

Education

Bachelor's degree in Social Work or similar professional qualifications; Current Social Work license as regulated by state law. Two years of social work supervised experience. Working directly with geriatric clients in a health care setting preferred.

Pay: $65,000.00 - $80,000.00 per year
